Receive referral
Complete our “Client Referral Form”. For NDIS clients, once received we will get to work to develop your Service Agreement.

Engagement
A member of the APR team will contact you to book your first appointment. For NDIS clients, we will also send you a Service Agreement for you to sign.

Timely Assessment
We think it is important that you receive great care, quickly. We organise to meet you quickly, so you can improve quickly and prevent problems that can occur by delaying your treatment.

Determination of Supports
Your first assessment with us will allow us to identify, and prioritise with you your specific goals and needs. Now we set about communicating these with those who need and you want to know- your family, insurers, and other supports and providers, such as your prosthetist.

Delivery
We work together to deliver on the goals you identified. These may change over time but with a commitment to communication and teamwork as a centrepiece of our relationship, our treatment evolves over time as you improve.

Lifelong Availability
Even after you meet your goals, we understand that living with amputation is a lifelong journey, and you know that we are never far away if you need advice or someone to talk to. Our phone stays open.
